Returning student workers need only sign and submit their Financial Aid Form and Student Worker Contract at the beginning of the fall semester. All other paperwork is kept on file with the Treasurer's Office.

Official Employment-Related Paperwork
ALL student workers, whether returning or new, need to complete paperwork prior to receiving payment for any work performed. This paperwork must be completed before you begin work:
**Office of Financial Aid (OFA Student Worker Application/OFA Student Worker Rights and Responsibilities – This form is obtained from and required by the OFA of ALL new and returning student workers. All students who have been awarded Institutional or Federal Aid will find a copy of this form in their campus mail box. If you don’t receive one, or have lost it, replacement forms may be obtained from the OFA. Review for correctness the information on the front of the form and be sure to sign and date the Rights & Responsibilities on the reverse side.
**W4 – Only new employees need to complete this form. This form is available from your supervisor.
**I9 – Only new students need to complete this form. This form is available from your supervisor.
--Copies of IDs are needed to prove citizenship and eligibility for employment and for completion of the federal I9 form. Suitable IDs include your driver’s license, Social Security Card, an active Passport, State ID, Student ID. Two are needed except in the case of a Passport, where that alone will do. Only new students need to complete this requirement.
**Confidentiality Form – All new students need to read and sign this form. Return signed form to your supervisor.
**Contract – All new and returning student workers should read and sign a copy at the beginning of the fall semester. Copies of this form can be obtained from and should be returned to your supervisor.
**LST tax – (Local Services Tax) This annual tax of $52.00 will be deducted from your pay in increments over the course of the year. If you have another job in Pennsylvania where this tax is being collected, you must submit an Application for Exemption Form before you begin work. If money has already been deducted, you must submit an Application For Refund From Local Services Tax form. Both of these forms are available at the Payroll Office. **Residency Certification Form - For new hires. Fill in top section. Go to www.newPA.com to determine your PSD code. EIT box does not have to be filled in. All student workers must sign and submit this form even if you live out of state.
**Direct Deposit Form (required for all student workers) available from your supervisor.
